Overview:
The Transplant Coordinator has the primary responsibility for coordinating all clinical aspects of care for patients in the pre, in-patient, and post transplant phases of the transplantation process. In addition, s/he provides direct patient care to patients who are awaiting or who have received a kidney, kidney/pancreas or pancreas transplant. This position maintains all policies and procedures, and works collaboratively with the Manager- Organ Transplant, Physicians, social worker, financial coordinator, and support staff to provide comprehensive care to the recipient.

Qualifications:
Education, Knowledge, Skills and Abilities Required: 1. Graduate of a NLN/AACN accredited program in nursing. 2. Bachelors degree in nursing. 3. Minimum 5 years of experience as a Registered Nurse in Acute Care setting. 4. Must have proven independent thinking, decision making, problem solving and prioritizing skills. 5. Excellent skills to communicate and interact effectively with all levels of staff, physicians, patients and external constituents, both verbally and in writing. 6. Excellent ability to multitask. 7. Proficient in Microsoft Word, Excel and other Office programs. 8. Membership in North American Transplant Coordinators Organization. 9. Certified Clinical Transplant Coordinator (CCTC) certification within 30 months of hire. Education, Knowledge, Skills and Abilities Preferred: 1. Masters degree in nursing. 2. 7 years in acute care medical/surgical setting. 3. Bilingual English/Spanish. 4. Previous experience working in a transplant center. 5. Critical care nursing experience. 6. Knowledge of Google suite platforms, Licenses and Certifications Required: 1. NJ State Professional Registered Nurse License. 2. AHA Basic Health Care Life Support HCP Certification. Licenses and Certifications Preferred: 1. Certified Clinical Transplant Coordinator.

Responsibilities:
1. Implements components of direct and indirect care to patients requiring advanced clinical expertise or sophisticated problem solving abilities. Develops care plans for patients who have complex needs or whose needs require complex interventions. 2. Collaborates with patient, family, and health care providers in achieving optimal patient outcomes. Serves as a liaison between physicians, patients, and non-clinical staff members in interpretation of issues pertinent to nursing. 3. Interprets and disseminates clinical findings to physicians and advanced practice practitioners 4. Assesses the suitability of and prepares potential transplant recipients for solid organ transplantation, applying recipient selection criteria. 5. Monitors and evaluates recipient health status and diagnostic tests on a regular basis and maintains appropriate records throughout all phases of the transplantation process 6. Coordinates evaluation and completes information/testing requirements for patient presentation at selection committee and communicates outcome to patient, and providers as needed. 7. Evaluates educational needs and provides nursing clinical education to recipients, families, and staff regarding expected outcomes, tissue typing, risks, complications and patient responsibilities during the transplantation process. Provides emotional support to family members. 8. Participates in and implements program QAPI initiatives and is an active member of QAPI meetings and Selection Committee meetings. 9. Carries out OPTN/UNOS listing of transplant candidates and completes documentation requirements as needed. 10. Adheres to policy regarding re-evaluation and communication with patients during the waitlist period. 11. Coordinates deceased donor transplant process in collaboration with on-call transplant surgeon during perioperative period (i.e., when organ becomes available to transplant candidate). Coordinates preoperative care for patients and monitors, evaluates and reports postoperative organ function. 12. Maintains communication with the patient -s physician; acts as liaison between the patient and patient -s family and other healthcare providers. 13. Serves as liaison and resource person for in-patient staff, residents and physicians. 14. Coordinates patient discharge after hospitalization and 15. Coordinates the care of the transplant recipient in the outpatient setting. 16. Utilizes the nursing process to plan, implement and coordinate patient care through collaboration with other healthcare team members. 17. Develops innovative activities and procedures that contribute to improvement in the delivery of patient care 18. Maintains professional competence by participating in continuing education through outside seminars, hospital in-services, or through formal on-going didactic education. 19. Collaborates with the training and education department in the development, implementation, and evaluation of continuing education programs as needed. 20. Maintains membership in professional organizations. 21. Keeps current with professional nursing practices and nursing research studies. 22. Participates in departmental research initiatives as appropriate. 23. May be required to perform other duties as assigned or when necessary. 24. Identifies the needs of the patient population served and modifies and delivers care that is specific to those needs (i.e., age, culture, language, hearing and/or visually impaired, etc.). This process includes communicating with the patient, parent, and/or primary caregiver(s) at their level (developmental/age, educational, literacy, etc.). 25. Other duties and/or projects as assigned. 26. Adheres to HMH Organizational competencies and standards of behavior.
