of $3000 , based on five 8-hour day shifts per week , providing an

Travel Home Heath Physical Therapist: Evaluate patients' physical
function and mobility to develop customized care plans Provide
therapeutic exercises, gait training, balance activities, and
functional mobility interventions in home settings Educate patients
and caregivers on safe mobility techniques and home exercise
programs Collaborate with physicians, nurses, and other members of
the care team to ensure coordinated care Monitor and document
patients' progress toward established goals and adjust treatment
plans as needed Maintain compliance with all documentation, safety,
and regulatory requirements Participate in interdisciplinary case
conferences and discharge planning as appropriate Requirements for
a Travel Home Heath Physical Therapist: Valid Physical Therapist
license in the state of West Virginia Doctorate or Master's degree
in Physical Therapy from an accredited program Minimum of 1 year of
recent experience in a Home Health setting BLS certification (AHA
or equivalent) Strong communication, problem-solving, and patient
