Job Description:
Job Description Job Description Objective : The purpose of the
Certified Homemaker Home Health Aide (CHHA) is to travel to the
clients home and provide a variety of services that allow
individuals needing care to remain in their homes. This care is
provided according to a written care plan and includes personal
care services and activities of daily living (ADLs) as well as,
light housekeeping, meal preparation, companionship, and assistance
with other instrumental activities of daily living (IADLs) to
assure a safe clean home environment. Primary Responsibilities: ·
Travel to client's home and provide the tasks as specified by the
written service plan. These activities require a variety of
physical demands as outlined in Working Conditions below, reliable
attendance at scheduled assignments, and the ability to read and
interpret the client's written care plan. · Assist the client with
Activities of Daily Living (ADLs) such as bathing, grooming,
toileting and incontinence care, transfers and ambulation, and
feeding. · Perform light housekeeping duties such as dusting,
laundry, vacuuming, general cleaning of bathroom, kitchen, and
living area when part of the written care plan. · Assist client
with planning, shopping, and preparing meals. Feed client or assist
with feeding if necessary and offer client fluids according to the
written service plan. Clean dishes, appliances, and kitchen work
area after preparation. · Observe client's condition or living
arrangements and report changes to the appropriate staff member.
This activity requires good judgment and observation skills. ·
Provide companionship. This activity requires a caring attitude,
courtesy, and patience. · Documentation of Activities completed,
arrival and departure times via telephony and or a scheduling app
completed by the end of each shift. Secondary Responsibilities: ·
Adherence to Agency policies and procedures. · Maintain a
professional image, good appearance, and person hygiene. · Be
punctual and maintain an excellent attendance record. · Maintain
client and agency confidentiality. · Report hours using the
telephony or app system. · Attend Agency meetings and training and
make learning a part of the job. · May require you to run errands
and provide incidental transportation for a client using your
vehicle or a clients vehicle · Perform other duties as assigned
Qualifications, Knowledge, Skills and Abilities: Hold a current,
valid Certified Homemaker Home Health Aide Certification issued by
the New Jersey Board of Nursing. Successful completion of the
mandated skills evaluation. Be a high school graduate, GED or
equivalent of life experience and education. Ability to read,
write, and speak English, follow written and verbal directions, and
maintain required records. Ability to lift, push or pull 25 pounds
Possess a valid drivers license and valid auto insurance Maturity
and dependability are required.
